Neuralgic Amyotrophy
Prioritize efficient, coordinated movement before progressing strengthening.
Neuralgic amyotrophy is an episodic neuritis characterized by severe neuropathic pain followed by weakness and muscle atrophy. It most commonly affects the brachial plexus, although the lumbosacral plexus and phrenic nerve can also be involved.
The exact cause remains unclear. Current evidence suggests a multifactorial process involving immune, mechanical, and genetic contributors.
Incidence

than women
Most recover
Important Clinical Pearl
Traditional strengthening may reinforce abnormal movement patterns and overwork weak or compensating muscles. Prioritize efficient coordinated movements first.
When diaphragmatic involvement is suspected, additional assessment such as spirometry or ultrasound may be indicated.
PT:
improve movement strategy
Better energy management
OT:
apply it to daily activities and work
More effective activity retraining
Patient:
better carryover into real life
Targeted ergonomic intervention
Many people with neuralgic amyotrophy can return to meaningful work. Persistent weakness and reduced endurance, however, may require temporary or longer-term accommodations.
